Take a look at this list of Insurance Plans for which we are/are not providers. If you don't see your plan listed, please call us or email us a picture of both sides of your card. We will happily investigate for you!
What should I do if I have an emergency? What if it is after hours?
For true emergencies, please dial 911. If you are having a fever or your throat is closing up in conjunction with a skin condition like a rash, please go to the hospital. For other urgent skin care needs, we can usually work patients in on a same day basis.
If it is after hours, please call (512) 786-3498 and follow the instructions detailed in the voice message to reach Dr. Farady.
I need to cancel my appointment. Will I be charged a fee?
Please notify our office at least 24 hours prior to your appointment if you need to cancel or reschedule. We do not currently have a cancellation fee, and we hope to maintain this policy. Missed filler appointments will result in the loss of the deposit.
I need a prescription refilled. What should I do?
If you have run out of authorized refills, you may call your pharmacy and have them fax a request to our office. We will respond to the request within 24 hours. Routine refills can only be filled during office hours.
Here are some reasons why a request may be denied:
- Refills cannot be authorized if more than one year has elapsed since your last visit.
- Refills may be denied if we feel that it is in your best interest to be seen first.
- Refills may be denied if there has been a missed appointment.
If your prescription requires a prior authorization, we will ask that you contact your insurance carrier and have them fax us any necessary forms.
Does Dr. Farady see children?
Dr. Farady sees children ages 12-17 years old. Minors must be accompanied by their parent or guardian to each visit.
Will I need to fill out paperwork for my visit?
We strive to keep the paperwork to a minimum. That being said, there is just no getting around it in the medical field. If you are an existing patient and haven’t been in since the previous year, we will give you one form to meet our HIPAA needs and to update your contact information. If you would like to fill it out ahead of time, you may find it on our New Patients page. It is simply the third page of the new patient paperwork, entitled "Payment Policy".
If you are a new patient, there are a few pages we ask you to complete. New patients are encouraged to either arrive 10 to 15 minutes prior to their appointment, or to fill these out ahead of time. If you are an established patient with Dr. Farady but are seeing an esthetician for the first time, we will give you one form to target your facial needs. These forms are all found on our New Patients page.
What should I bring to my appointment?
Please bring your photo ID and most recent health insurance card/s to each visit. Please bring a list of your medical conditions and medications with you to your visit. This is especially important for any creams, ointments, or lotions you have been prescribed for your skin condition. Also, our A.C. works very well – we recommend bringing a sweater!
What is your policy regarding payments?
We will file insurance claims with plans for which we are participating providers. We do collect co-pays, co-insurance and unmet deductibles on the day of your visit. Many of our services, including office surgeries, skin biopsies, and freezing of warts and other skin lesions, are subject to co-insurance and deductibles. If you are unsure of your insurance coverage, you may contact our billing department for assistance prior to your visit. Some of our services may be considered not medically necessary, and therefore not covered by insurance. You may want to check with your insurance carrier prior to your visit.
For private pay patients, full payment is expected on the day of service unless prior arrangements have been made. Billing information will be provided to you on request if you wish to file for reimbursement from your insurance carrier.
*If current or correct insurance information is not provided, we will make every attempt to file the claim for payment, but if the claim is denied, the patient will be responsible for payment of services rendered.
A $100.00 non-refundable deposit will be required to schedule a filler appointment.
What methods of payment do you accept?
We accept Visa, Mastercard, Discover and American Express. We also accept cash and checks. We do not take Care Credit.
