Are you a provider for my insurance plan?
Current insurance providers we accept are:

What is your policy regarding billing/payments?
We will file insurance claims with plans for which we are participating providers. If you are unsure of your insurance coverage, you may contact our billing department directly at 813-333-1528 for assistance prior to your visit. Some of our services may be considered not medically necessary, and therefore not covered by insurance. You may want to check with your insurance carrier prior to your visit.

For private pay patients, full payment is expected on the day of service unless prior arrangements have been made. Billing information will be provided to you on request if you wish to file for reimbursement from your insurance carrier.

Dr. Farady has teamed up with Palm Harbor Dermatology, who now handles our billing. You may receive an invoice from them directly, depending on services not covered by your insurance plan.

What should I do if I have an emergency? What if it is after hours?
For true emergencies, please dial 911, or go to your nearest hospital. For other urgent skin care needs, we can usually work patients in on a same day basis.

If it is after hours, please call (512) 786-3498 and follow the instructions detailed in the voice message to reach Dr. Farady.

I need to cancel my appointment. Will I be charged a fee?
When possible, please notifiy our office at least 24 hours prior to your appointment if you need to cancel or reschedule. If an appointment is missed we charge a $25 cancellation fee. Missed filler appointments will result in the loss of the deposit.

I need a prescription refilled. What should I do?
If you have run out of authorized refills, you may call your pharmacy and have them fax a request to our office. We do our best to respond to all requests within 24 hours, when possible. Routine refills can only be filled during office hours.

If your prescription requires a prior authorization, we will ask that you contact your insurance carrier and have them fax us any necessary forms. Refills cannot be authorized if more than one year has elapsed since your last visit.

Does Dr. Farady see children?
Dr. Farady sees children ages 13 years old and older. Minors must be accompanied by their parent or guardian to each visit.

What should I bring to my appointment?
Please bring your photo ID and most recent health insurance card/s to each visit, along with a list of your medical conditions and medications. This is especially important for any creams, ointments, or lotions you have been prescribed for your skin condition. 

What methods of payment do you accept?
Unfortunately we do not accept cash or checks, however, we gladly accept Visa, Mastercard, Discover and American Express.
